Analyser l'hétérogénéité des perturbations métaboliques du cerveau dans le '"trouble du spectre de l'autisme"

CONTEXTE:
Malgré la prévalence croissante des troubles du spectre de l'autisme (TSA), ses bases cérébrales restent incertaines. Des taux anormaux de composés N-acétyle, de glutamate + glutamine, de créatine + phosphocréatine ou de composés de choline mesurés par spectroscopie à résonance magnétique proton suggèrent que la densité neuronale ou gliale, le métabolisme énergétique mitochondrial et / ou l’inflammation contribuent à la neuropathologie des TSA. La distribution neuroanatomique de ces métabolites pourrait aider à évaluer les théories principales du TSA. Cependant, la plupart des études antérieures de spectroscopie par résonance magnétique avaient de petits échantillons (tous <60, la plupart <20), n’interrogeaient qu’une petite fraction du cerveau et évitaient d’évaluer les effets de l’âge, du sexe et du QI.
METHODES:
Nous avons acquis la spectroscopie par résonance magnétique presque complète du cerveau de composés N-acétyle, glutamate + glutamine, créatine + phosphocréatine et choline chez 78 enfants et adultes avec un TSA et 96 enfants et adultes au développement typique, en évaluant de manière rigoureuse les effets du diagnostic et de la gravité métabolites, modérés par l'âge, le sexe et le QI.
RÉSULTATS:
Les effets des TSA et leur gravité incluaient une réduction des taux de multiples métabolites dans la substance blanche et le cortex périsylvien et des taux élevés dans le cingulaire postérieur, conformes aux théories de la substance blanche et du socio-cerveau du TSA. Au niveau local, des diminutions plus lentes et plus rapides des métabolites avec l’âge ont été observées entre TSA et TD. Les différences de métabolites entre les hommes et les femmes étaient considérablement plus faibles chez les TSA que les enfants et les adultes au développement typique. Des diminutions des métabolites avec diminution du QI spécifiques au TSA sont survenues dans plusieurs régions du cerveau.
CONCLUSIONS:
Les résultats corroborent la densité neuronale ou gliale anormale multifocale, l'énergétique mitochondriale ou la neuroinflammation chez les TSA, ainsi que les effets modérateurs extrêmement atypiques et généralisés de l'âge, du sexe et du QI. 
Ces résultats aident à analyser la signature neurométabolique des TSA par hétérogénéité phénotypique.

Abstract

BACKGROUND:

Despite rising prevalence of autism spectrum disorder (ASD), its brain bases remain uncertain. Abnormal levels of N-acetyl compounds, glutamate+glutamine, creatine+phosphocreatine, or choline compounds measured by proton magnetic resonance spectroscopy suggest that neuron or glial density, mitochondrial energetic metabolism, and/or inflammation contribute to ASD neuropathology. The neuroanatomic distribution of these metabolites could help evaluate leading theories of ASD. However, most prior magnetic resonance spectroscopy studies had small samples (all <60, most <20), interrogated only a small fraction of the brain, and avoided assessing effects of age, sex, and IQ.

METHODS:

We acquired near-whole-brain magnetic resonance spectroscopy of N-acetyl compounds, glutamate+glutamine, creatine+phosphocreatine, and choline compounds in 78 children and adults with ASD and 96 typically developing children and adults, rigorously evaluating effects of diagnosis and severity on metabolites, as moderated by age, sex, and IQ.

RESULTS:

Effects of ASD and its severity included reduced levels of multiple metabolites in white matter and the perisylvian cortex and elevated levels in the posterior cingulate, consistent with white matter and social-brain theories of ASD. Regionally, both slower and faster decreases of metabolites with age were observed in ASD versus TD. Male-female metabolite differences were widely smaller in ASD than typically developing children and adults. ASD-specific decreases in metabolites with decreasing IQ occurred in several brain areas.

CONCLUSIONS:

Results support multifocal abnormal neuron or glial density, mitochondrial energetics, or neuroinflammation in ASD, alongside widespread starkly atypical moderating effects of age, sex, and IQ. These findings help parse the neurometabolic signature for ASD by phenotypic heterogeneity.

Devrions-nous envisager la compartimentation sous-cellulaire des métabolites et, dans l’affirmative, comment les mesurer?

OBJET DE L’EXAMEN:
Pour examiner les conséquences du métabolisme compartimenté au niveau subcellulaire, fournissez des exemples prototypiques du métabolisme compartimenté et décrivez les méthodes permettant d'examiner le métabolisme compartimenté.
CONCLUSIONS RÉCENTES:
Les progrès de la métabolomique et du traçage isotopique ont mis en évidence l’importance des compartiments subcellulaires du métabolisme. La découverte des effets biologiques des métabolites en tant qu'intermédiaires bioénergétiques, blocs de construction anaboliques, médiateurs de signalisation et effecteurs dans les modifications post-traductionnelles des protéines et des acides nucléiques a mis en évidence le rôle de la compartimentation dans la détermination du devenir métabolique. Les progrès récents des méthodes directes et indirectes de quantification du métabolisme compartimenté ont amélioré les approches historiques. Des capteurs de métabolites génétiquement codés, des sondes chimiques, une purification par immunoaffinité et une modélisation métabolique résolue en compartiments ont récemment été appliqués pour étudier la compartimentation.

Abstract

PURPOSE OF REVIEW:

To examine the consequences of metabolism compartmentalized at the subcellular level, provide prototypical examples of compartmentalized metabolism, and describe methods to examine compartmentalized metabolism.

RECENT FINDINGS:

Progress in metabolomics and isotope tracing has underscored the importance of subcellular compartments of metabolism. The discovery of biological effects of metabolites as bioenergetic intermediates, anabolic building blocks, signaling mediators, and effectors in posttranslation modifications of proteins and nucleic acids have highlighted the role of compartmentalization in determining metabolic fate. Recent advances in both direct and indirect methods to quantify compartmentalized metabolism have improved upon historical approaches. Genetically encoded metabolite sensors, chemical probes, immunoaffinity purification, and compartment-resolved metabolic modeling have all been recently applied to study compartmentalization.

SUMMARY:

Accurate measurement of metabolites in distinct subcellular compartments is important for understanding and pharmacologically targeting metabolic pathways in diverse disease contexts, including cancer, diabetes, heart failure, obesity, and regulation of the immune system. Direct and indirect approaches to quantify compartmentalized metabolism are advancing rapidly. Yet, major challenges remain in the generalizability, rigor, and interpretation of data from the available methods to quantify compartmentalized metabolism.

Développements émergents dans la recherche sur le microbiome et la microglie: implications pour les troubles neurodéveloppementaux

De l'immunologie à la neuroscience, les interactions entre le microbiome et l'hôte sont de plus en plus appréciées en tant que moteurs puissants de la santé et de la maladie. Des études épidémiologiques ont précédemment identifié des corrélations convaincantes entre les atteintes périnatales du  microbiome et les résultats neurocomportementaux, dont les détails mécaniques commencent à peine à prendre forme grâce à des modèles animaux sans germes et basés sur les antibiotiques.  
Cette revue résume les développements parallèles des recherches cliniques et précliniques qui suggèrent des rôles neuroactifs pour les bactéries intestinales et leurs métabolites.  
Les chercheurs examinent également le domaine naissant de la recherche sur la diaphonie microbiologique-microglie, qui comprend des stratégies pharmacologiques et génétiques pour informer les capacités fonctionnelles de la microglie en réponse à la programmation microbienne. Enfin, ils abordent une hypothèse émergente derrière les troubles neurodéveloppementaux, qui implique la dysbiose du microbiome dans la programmation atypique des cellules neuro-immunes, à savoir la microglie.

Abstract

From immunology to neuroscience, interactions between the microbiome and host are increasingly appreciated as potent drivers of health and disease. Epidemiological studies previously identified compelling correlations between perinatal microbiome insults and neurobehavioral outcomes, the mechanistic details of which are just beginning to take shape thanks to germ-free and antibiotics-based animal models. This review summarizes parallel developments from clinical and preclinical research that suggest neuroactive roles for gut bacteria and their metabolites. We also examine the nascent field of microbiome-microglia crosstalk research, which includes pharmacological and genetic strategies to inform functional capabilities of microglia in response to microbial programming. Finally, we address an emerging hypothesis behind neurodevelopmental disorders, which implicates microbiome dysbiosis in the atypical programming of neuroimmune cells, namely microglia.

Classification de haute efficacité des enfants avec "trouble du spectre de l'autisme"que

Le " trouble du spectre de l'autisme" (TSA) est un vaste ensemble de troubles développementaux avec divers symptômes et degrés d'incapacité. Actuellement, les TSA sont principalement diagnostiqués avec des outils psychométriques, souvent incapables de fournir un diagnostic précoce et fiable. Récemment, des méthodes biochimiques ont été explorées pour répondre à ce dernier besoin. Par exemple, une augmentation de la prédisposition aux TSA a été associée à des anomalies des métabolites dans le métabolisme du carbone mono-dépendant des folates (FOCM) et la transsulfuration (TS).  
Des métabolites multiples dans les voies FOCM / TS ont été mesurés, et des outils d'analyse statistique ont été utilisés pour identifier certains métabolites étroitement apparentés aux TSA. La principale difficulté de ces études biochimiques provient:
(i) de la détermination inefficace des métabolites les plus importants et 
(ii) de la compréhension de la relation entre ces métabolites et les TSA.  
Cet article présente une nouvelle méthode basée sur les résultats obtenus dans la modélisation de Support Vector Machine (SVM) combinée à l'analyse de sensibilité de la représentation de modèle à haute dimension (HDMR). La nouvelle méthode identifie de manière efficace et efficiente les principaux métabolites responsables dans les voies FOCM / TS, classe leur importance et découvre leurs modes d'action indépendants et corrélatifs sur les TSA.  
Une telle information est utile non seulement pour jeter les bases d'une interprétation pathologique, mais aussi pour fournir un diagnostic précoce et fiable, conduisant idéalement à un traitement complet subséquent des TSA. Avec seulement quelques dizaines de passages du modèle SVM, la nouvelle méthode peut identifier les combinaisons des métabolites les plus importants dans les voies FOCM / TS qui conduisent à des TSA. Les efforts antérieurs pour trouver ces métabolites ont nécessité des centaines de milliers de séries de modèles avec les mêmes données.

High efficiency classification of children with autism spectrum disorder

Abstract

Autism spectrum disorder (ASD) is a wide-ranging collection of developmental diseases with varying symptoms and degrees of disability. Currently, ASD is diagnosed mainly with psychometric tools, often unable to provide an early and reliable diagnosis. Recently, biochemical methods are being explored as a means to meet the latter need. For example, an increased predisposition to ASD has been associated with abnormalities of metabolites in folate-dependent one carbon metabolism (FOCM) and transsulfuration (TS). Multiple metabolites in the FOCM/TS pathways have been measured, and statistical analysis tools employed to identify certain metabolites that are closely related to ASD. The prime difficulty in such biochemical studies comes from (i) inefficient determination of which metabolites are most important and (ii) understanding how these metabolites are collectively related to ASD. This paper presents a new method based on scores produced in Support Vector Machine (SVM) modeling combined with High Dimensional Model Representation (HDMR) sensitivity analysis. The new method effectively and efficiently identifies the key causative metabolites in FOCM/TS pathways, ranks their importance, and discovers their independent and correlative action patterns upon ASD. Such information is valuable not only for providing a foundation for a pathological interpretation but also for potentially providing an early, reliable diagnosis ideally leading to a subsequent comprehensive treatment of ASD. With only tens of SVM model runs, the new method can identify the combinations of the most important metabolites in the FOCM/TS pathways that lead to ASD. Previous efforts to find these metabolites required hundreds of thousands of model runs with the same data.

Abstract

BACKGROUND:

The diagnosis of autism spectrum disorder (ASD) at the earliest age possible is important for initiating optimally effective intervention. In the United States the average age of diagnosis is 4 years. Identifying metabolic biomarker signatures of ASD from blood samples offers an opportunity for development of diagnostic tests for detection of ASD at an early age.
Le diagnostic de trouble du spectre autistique (TSA) au plus jeune âge possible est important pour démarrer une intervention d'une efficacité optimale. Aux États-Unis, l'âge moyen du diagnostic est de 4 ans. Identifier les signatures de biomarqueurs métaboliques de TSA à partir d'échantillons de sang offre une opportunité pour le développement de tests de diagnostic pour la détection de TSA à un âge précoce.

OBJECTIVES:

To discover metabolic features present in plasma samples that can discriminate children with ASD from typically developing (TD) children. The ultimate goal is to identify and develop blood-based ASD biomarkers that can be validated in larger clinical trials and deployed to guide individualized therapy and treatment.

METHODS:

Blood plasma was obtained from children aged 4 to 6, 52 with ASD and 30 age-matched TD children. Samples were analyzed using 5 mass spectrometry-based methods designed to orthogonally measure a broad range of metabolites. Univariate, multivariate and machine learning methods were used to develop models to rank the importance of features that could distinguish ASD from TD.

RESULTS:

A set of 179 statistically significant features resulting from univariate analysis were used for multivariate modeling. Subsets of these features properly classified the ASD and TD samples in the 61-sample training set with average accuracies of 84% and 86%, and with a maximum accuracy of 81% in an independent 21-sample validation set.
Un ensemble de 179 caractéristiques statistiquement significatives résultant de l'analyse univariée ont été utilisées pour la modélisation multivariée. Les sous-ensembles de ces traits classe correctement les échantillons de TSA et TD dans l'ensemble de la formation 61échantillons avec une précision moyenne de 84% et 86%, et avec un maximum de précision de 81% dans en ensemble indépendant de 21 échantillons de  validation.

CONCLUSIONS:

This analysis of blood plasma metabolites resulted in the discovery of biomarkers that may be valuable in the diagnosis of young children with ASD. The results will form the basis for additional discovery and validation research for 1) determining biomarkers to develop diagnostic tests to detect ASD earlier and improve patient outcomes, 2) gaining new insight into the biochemical mechanisms of various subtypes of ASD 3) identifying biomolecular targets for new modes of therapy, and 4) providing the basis for individualized treatment recommendations.
Cette analyse des métabolites plasmatiques de sang a permis la découverte de biomarqueurs qui peuvent être utiles dans le diagnostic des jeunes enfants avec TSA. 
Les résultats serviront de base pour la découverte supplémentaire et la recherche de validation pour:

Abstract

Autism spectrum disorders (ASDs) are a group of neurodevelopmental disorders which have a severe life-long effect on behavior and social functioning, and which are associated with metabolic abnormalities. (Les Troubles du Spectre Autistique (TSA) sont un groupe de troubles neurologiques du développement qui ont un effet sévère tout au long de la vie sur le comportement et le fonctionnement social, et qui sont associés à des anomalies métaboliques.)
Their diagnosis is on the basis of behavioral and developmental signs usually detected before three years of age, and there is no reliable biological marker. The objective of this study was to establish the volatile urinary metabolomic profiles of 24 autistic children and 21 healthy children (control group) to investigate volatile organic compounds (VOCs) as potential biomarkers for ASDs.

Solid-phase microextraction (SPME) using DVB/CAR/PDMS sorbent coupled with gas chromatography-mass spectrometry was used to obtain the metabolomic information patterns. Urine samples were analyzed under both acid and alkaline pH, to profile a range of urinary components with different physicochemical properties. Multivariate statistics techniques were applied to bioanalytical data to visualize clusters of cases and to detect the VOCs able to differentiate autistic patients from healthy children. In particular, orthogonal projections to latent structures discriminant analysis (OPLS-DA) achieved very good separation between autistic and control groups under both acidic and alkaline pH, identifying discriminating metabolites.

Among these, 3-methyl-cyclopentanone, 3-methyl-butanal, 2-methyl-butanal, and hexane under acid conditions, and 2-methyl-pyrazine, 2,3-dimethyl-pyrazine, and isoxazolo under alkaline pH had statistically higher levels in urine samples from autistic children than from the control group. Further investigation with a higher number of patients should be performed to outline the metabolic origins of these variables, define a possible association with ASDs, and verify the usefulness of these variables for early-stage diagnosis.
