Abstract

We conducted meta-analyses of 67 studies on the association between neonatal proxies of impaired gas exchange and intellectual disability (ID) or autism spectrum disorders (ASD). Neonatal acidosis was associated with an odds ratio (OR) of 3.55 [95 % confidence interval (95 % CI) 2.23-5.49] for ID and an OR of 1.10 (95 % CI 0.91-1.31) for ASD. Children with a 5-min Apgar score of <7 had an OR of 5.39 (95 % CI 3.84-7.55) for ID and an OR of 1.67 (95 % CI 1.34-2.09) for ASD. O2 treatment was associated with an OR of 4.32 (95 % CI 3.23-5.78) for ID and an OR of 2.02 (95 % CI 1.45 to 2.83) for ASD. Our meta-analysis demonstrates an increased risk of ID and (to a lesser extent) ASD in children with neonatal hypoxia. Moreover, our findings raise the possibility that concomitant ID might account for the observed association between the gas exchange proxies and ASD.
Nous avons effectué des méta-analyses de 67 études sur l'association entre les intermédiaires néonatals d'échange de gaz réduit et la déficience intellectuelle (DI) ou le trouble du spectre de l'autisme (TSA). L'acidose néonatale a été associée à un risque relatif rapproché (OR) de [95% DI  (IC à 95%) de 2,23 à 5,49] 3,55 pour les ID et un OR de 1,10 (IC à 95% 0,91 à 1,31) pour les TSA. Les enfants avec un score d'Apgar 5-min < 7 avaient un OR de 5,39 (IC à 95% 3,84 à 7,55) pour les ID et un OR de 1,67 (IC à 95% 1,34 à 2,09) pour les TSA.Le traitement à l'O² a été associé avec un OR de 4,32 (IC à 95% 3,23 à 5,78) pour les DI et un OR de 2,02 (IC à 95% 1,45 à 2,83) pour les TSA.  
Notre méta-analyse démontre un risque accru de DI et (dans une moindre mesure) de TSA chez les enfants avec une hypoxie néonatale. En outre, nos résultats soulèvent la possibilité qu'une DI concomitante pourrait expliquer l'association observée entre les intermédiaires d'échange gazeux et le TSA.

Abstract

Perinatal and background risk factors for autism were identified in a cohort of autistic children in Zhengzhou, China, to formulate preventative and treatment strategies for high-risk families. 
Les facteurs de risque périnatals et environnementaux de l'autisme ont été identifiés dans une cohorte d'enfants autistes à Zhengzhou, en Chine, afin de formuler des stratégies de prévention et de traitement pour les familles à haut risque. 
In this case-control study, children were screened for suspected autism using the Autism Behavior Checklist (ABC) and diagnosed according to DSM-IV and the Childhood Autism Rating Scale (CARS). We collected perinatal histories and clinical data of 286 confirmed autistic children treated at the Third Affiliated Hospital Children׳s Psychological Clinic of Zhengzhou University from 2011 to 2013. The control group consisted of 286 healthy children from area kindergartens. Maternal age>30 years, parental introversion as measured by the Eysenck Personality Questionnaire, low level of parental education, smoking, abortion threat, pregnancy complications, maternal illness during pregnancy, maternal mental health, family history of mental illness, neonatal jaundice, birth asphyxia, premature rupture of the fetal membrane, and gestational age<37 weeks were significantly higher in the autism group. These factors were significantly correlated with behavioral symptoms as measured by ABC scores (Kendall rank correlation). 
Birth asphyxia, neonatal jaundice, maternal age, parental introversion, family history of mental illness, abortion threat, premature delivery, and smoking were identified as independent risk factors by multivariate logistic regression.
L'asphyxie à la naissance, l'ictère néonatal, l'âge maternel, l'introversion des parents, les antécédents familiaux de maladie mentale, menace d'avortement, l'accouchement prématuré et le tabagisme ont été identifiés comme des facteurs de risque indépendants par régression logistique multivariée.
